We do offer personal blending for signature scents, we can create one of a kind all natural products for your clients or if a client has a special need we can create or help you create a blend for them. We can also do private labeling and bottling for businesses. We also offer staff training, including lectures or the Clinical Aromatherapy series.

Billing and Shipping addresses

Phone, Cell and Fax numbers

Credit Card number to keep on file

A physical copy of your State Sales Tax Number, the Resale License number.

 

You can fax, scan and email or mail this to us. You can now also sign up and enter all this info on the website @ www.botanicallyrootedbusiness.com.I will need this in order to open the account.

You can go to your states website and apply for this number. If you live in a state which does not require this, I will need you to call me @ 336-294-7727.
